For Surrey District Staff - How to Make a Referral

  • WHO IS ELIGIBLE?
    The service is offered to students with non-contagious illness or other medical reasons who expect to be absent for more than five consecutive days.
  • HOW DO YOU MAKE A REFERRAL?

    In FirstClass, select:
    SD36 Staff Conferences --> Student Support --> Hospital Homebound Conf --> Online HH Referral Form
    (SPECIAL NOTE: DO NOT USE THE ENTER/RETURN key while in the referral form - doing so will automatically send the incomplete referral to the HH coordinator - thank you.)

    The following information MUST be entered:
    • student’s name
    • student’s address
    • student’s phone number
    • student’s grade
    • parent(s) name
    • school name
    • school phone number
    • teacher(s) and courses taken (in secondary schools)
    • reason for absence - choose from: accident, surgery, mononucleosis, infection, disease, cancer, pregnancy, mental health or other.
    • name of person making the referral
    • doctor’s name, phone number, fax number (if known)

    Once the above information is completed, click Submit to send.
